What Is It

Exercise ECG for your CAA medical

When your AME or the CAA asks for an exercise ECG, they need it done a particular way. The CAA publishes a standard protocol, agreed with its cardiology advisers, and a test that does not follow it may have to be repeated.

We run every CAA exercise ECG to that protocol: the Bruce treadmill protocol, a 12-lead ECG recorded at the end of each minute and through ten minutes of recovery, and a symptom-limited test aiming for at least nine minutes of walking.

Every test is reported by our consultant cardiologist, and you receive the full report and ECG tracings within 24 working hours, ready to send to your AME.

When your AME may ask for one

  • Bundle branch block or other ECG findings
  • Follow-up after a heart event or stent
  • Investigation of fainting (syncope)
  • Heart valve or heart muscle conditions
  • ATCO Class 3 medical at age 65

Common Questions

Frequently asked questions

Everything you need to know about your appointment with The Heart Clinic.

Most pilots never need one. Your AME or the CAA will ask for an exercise ECG when it is needed, for example at the Class 1 extended cardiovascular assessment (first revalidation or renewal after age 65, then every 4 years), when your cardiovascular risk score is raised, after certain resting ECG findings such as bundle branch block, after a heart event or stent, or when investigating fainting. Air traffic controllers (Class 3) need an exercise ECG or equivalent at 65 whatever their risk score.
Yes. We follow the CAA's published exercise ECG protocol: the Bruce treadmill protocol, a 12-lead ECG recorded at the end of every minute of exercise and for ten minutes of recovery, and a symptom-limited test (you keep going until you cannot carry on, rather than stopping at a target heart rate).
For certification the CAA expects at least nine minutes of walking on the Bruce protocol. You walk rather than run and try not to hold the rails. If you stop before nine minutes, our consultant cardiologist states the reason in the report, as the CAA requires.
Do not stop any medication on your own. The CAA says beta blockers may sometimes be stopped for the test, but only after discussion with, and under the supervision of, your own doctor. We record whether you take beta blockers and whether they were stopped, as the protocol requires.
For Class 1 and Class 3 the CAA asks for the full exercise ECG report with the ECG tracings. For Class 2 and LAPL a detailed report is usually enough, although your AME may ask for the tracings too. We give you both the consultant cardiologist's report and the full tracings as standard, so you can forward them to your AME or GP (or to the CAA if they have asked you to send results directly).
No. We are a private cardiac diagnostics clinic. We perform and report the exercise ECG to the CAA protocol; your AME and the CAA assess the results and make the decision about your medical certificate.
No referral is needed. If your AME or the CAA has written to you asking for the test, please send us a copy so our report answers exactly what they have asked.
The exercise ECG is coming soon to our Luton clinic at 7 Wellington Street, LU1 2QH, close to Luton Airport. Get in touch to register your interest and we will contact you as soon as bookings open.
